Photograph, Film, Video and Audio Consent and Release

I hereby grant to Smith Health, Inc., its affiliates, subsidiaries, representatives and assigns, employees, or any person or persons acting under its permission or authority (collectively, “Smith”), the right and permission to:

1. film, photograph, or otherwise make audio or video recordings of me at one or more Event(s) hosted by Smith in calendar year 2025. ; and

2. publish, scan, display, broadcast, reproduce, edit, license, distribute, and otherwise use the resulting still or moving photographic or sound recordings of me (collectively, the “Reproduction”), with or without my name, in whole or in part, in any media now known or hereinafter devised, throughout the world, in such manner, for such purposes, and with such frequency as Smith shall determine in its sole discretion, without further consideration to me and without further authorization by me.

I acknowledge that the Reproduction shall constitute the sole property of Smith and may be used and displayed without restriction. I hereby waive any and all rights to compensation for, or ownership of, the Reproduction, and any and all rights of inspection or approval with regards to any proposed printed, audio or video publication and/or other use of the Reproduction. In addition, I hereby hold harmless and release and forever discharge Smith and its employees, agents, representatives, and licensees, from and against any and all claims, demands, and causes of action which I or my respective heirs, executors, administrators or assigns have or may have arising from or relating to this Consent and Release.

I hereby warrant that I am over eighteen years of age, and competent to contract in my own name. I have read this Consent and Release before affixing my signature below, and warrant that I fully understand the contents thereof.
